Population Norms Survey Consent

You have been randomly selected to be invited to take part in a survey to understand the health of Bermudians. Your participation in this study is completely voluntary, and you can opt out of the survey at any time.

In this survey we will ask you some questions about your health. Your information will be used anonymously with similar information from over 1,000 adults. This project will help policy makers in Bermuda to design and adapt health services for Bermuda. It will also provide healthcare providers with new tools to consider and discuss treatment options with patients in Bermuda.

The study is being conducted by the University of the West Indies (UWI) and it is funded by the Bermuda Health Council. At UWI we have done studies like this one in other countries. Your participation will just be to take this on-line survey which will take about 5 minutes.

There is no risk to you in this survey. If you are uncomfortable with any of the questions you can opt out of the survey at any time. Your participation in this survey will help the health system in Bermuda which will help everyone. Your responses are collected and analyzed anonymously. The report from this project will be based on the responses of all 1,000 people together so no one will see any person’s answers. This study has received ethical approval from the Ministry of Health in Bermuda and from the University of the West Indies.
